Also please remember to bring the following to your appointment:

·      Your insurance card

·      Photo ID*

·      A list of medications

·      Your preferred pharmacy information (fax number, address)

or a prescription bottle

·      A referral if needed

·      Payment if one applies

*Due to a new federal law designed to protect consumers from identity theft, a photo id is required.
